545i shake wen cold start

Suspect the spark plug wire connection maybe your culprit and double check they are all seated properly, are you fault codes pointing to any particular cylinder? If so start there.

The increased emission warning light stopped coming up after I changed the voltage regulator on my alternator and cleared the faults.
I have this same issue with my 04 545i....it drives me nuts because when I leave for work every morning, it sounds and looks like im driving some beater that's about to break down. However by the time Im out of my apartment complex and onto the main roads, the issue is gone, and idle is smooth (smooth is relative on my 545i)
The initial vibrations, sputtering and shaking are so bad that my wife has been completely scared away from driving the car.(not quite complaining as on several occasions Ive exaggerated issues to keep her from driving)
@2004_545i: is it possible to clean the secondary air intake if it is indeed clogged?

In the example of the OP it is clearly something related to changing plugs and coils. I bet they didn't replace the coils even though they said they did. If you waited until 100k miles to replace the plugs I have noticed on these engines that if you disturb the coil packs then reinstall them they can die. Stupid I know, but I has happened multiples to me on multiple BMW's. If you have to touch them over 60k miles replace them.
