I am just under 50K miles. I bought the car from my stepson so I know its history, which is stellar, no problems that haven't been corrected by software upgrades. I work from home and doubt I will put even as much as 10K/year on the car (probably more like 7-8K). I think I am better off keeping the $3K+ in the bank to cover possible repairs than to spend it on a warranty. Like others have said, each individual situation will vary.

Does anyone has inputs on whether or not it's worth it to get the extended maintenance program?

Thanks!
I think that is worse than the extended warranty... (* again situation dependent *)

$1,645.00 for 2 years and 50,000 miles... for me I dont put enough miles on the car. so that would mean 1 oil change each year, a brake flush and possibly brake replacement.

So that'll be $200 for oil changes
$400 for brake flush
$1200 for all the brakes for a total of $1800, and thats ONLY if the iDrive states that I need brakes for all the wheels. If it calculates and says I dont then I wont even get my money's worth for the extended maintenance.
There are no other maintenance that will be required between years 4 and 6 for me.

So for HIGH milage drivers in 50K miles you can potentially have:

4 oil changes = $500
brake flush = $400 <--- I'm just guessing I have no idea the price. (brake flushes are every 2 years)
complete brake change = $1200 once, I cant imagine you'll need two sets of complete brake changes in 50K miles unless you really drive like mad.
Air Filters? = $60 parts from dealer or less from internet and its a VERY easy DIY so I'm not even going to put labor into the price.

what else does the car need servicing? Oh spark plugs... if you can drive 100K miles before year 6 then you can probably get a new set of spark plugs so thats some money too... check the math and see if its worth it for you.

Someone (no not me) should start a poll and see how many people actually took full advantage of the extended warranty... and i'm not just saying using it once or twice but actually have used it enough to make up for the cost of the warranty.. whether on a E60 or prior BMWs...
I had an automatic transmission failure a little over a month after my original factory warranty expired. I had to have a full transmission replacement. But luckily I had the extended warranty. I didn't pay a penny for the repair, most likely covered the cost of the extended warranty.
