Changing Gears Problem

From: Los Angeles CA,
for the past month ive been noticeing this when im going up a hill the rpm goes on 3000 and stays there and when i give it more throttle it just goes to 4000 rpm it seems its having problem changing gears anyone heard of anything like this before
