Take a look at the picture closely. Seems like a piece of the loading ramp snapped off. It's typically equipment failure that causes these things. Have some faith in humanity

I saw that as well, and recognized it as the piece that articulates downward and touches the street to provide a smooth/level egress. However, that piece is not designed as a "backstop" or gate, therefore the car should have been secured otherwise, and certainly not on top of it until the hydraulic system had lowered the car down as far as it could.

Although not exactly the same, you can see that piece deployed in this MB photo, but the cars aren't rolling off yet, and still have the tie-down straps in place. Everybody makes mistakes.
