How hard is it to install the spacer? Can a qualified tire shop do it or does it require welding/etc?

Are there any adverse effects upon driving or tire wear from using a spacer?

Spacers slide on , just make sure you have wheel bolts that are long enough as the stock ones you have now wont be and use Anit seize on the back of the spacer so it does not get stuck on the hub. I have 15mm Rogue spacers in the rear and it looks great with no downside besides more money spent!
