exhaust blowing 530d

hi all
at low revs sounds like exhaust is blowing, or has a hole in it as its got a deep sound.
checked main exhaust and all seems fine. only thing i can think of is the exhaust manifold
anyone had similar problems?
mpg is around 23mpg locally
