From what you are describing your car is going in to limp mode (safe mode) as you said, the smell of diesel under the hood will most likely be related. incidentally a leaking inlet manifold will not cause the diesel smell no fuel goes through the intake manifold. You may have an injector leaking or something wrong in the fueling system. I would advise you to see a specialist asap rather than guessing and continuing to drive the car. The common rail diesel engines run with an extremely high fuel pressure.
Just wanted to add, I dont mean to sound alarmist but I personally dont take any chances with fuel.

I think the diesel smell is actually the big square gaskets that connect the intake manifold to the engine. I know the intake manifold should be full of air not diesel but I've seen a few E60 diesels with oily (diesely) engines below the swirl flaps. I think the swirl flap spindles or the manifold gaskets seep a little under full boost and strangely, somehow, diesel seeps out. Perhaps some kind of blow back?
I have got the car booked in to get the codes read but my current thinking is:
a) Intake manifold leak under full boost, detected by the engine management
b) Vacuum leak when a particular actuator is operated, detected by engine management

I guess you may be right but the oil that is in the intake is from the crankcase ventilation valve, I had a leak on my inlet manifold and it didnt cause the car to go in to limp mode.
I fitted a tuning box to a Volvo S60 D5 i used to own, it was great at first but then the car started going in to limp home mode. I have a code reader and the fault code was fuel leak small, i reset the code and removed the box and the car was fine. I never used the box again after that, beware of these things they are substitute for a remap. In your case I thought the iDrive would tell you that the engine was running on reduced power or something, i have seen other guys mention this.
Good luck getting it sorted and please share what it turns out to be.

Got the codes read:
4242 Glow Plug Cylinder 4
4262 Glow Plug Cylinder 6
428B Boost Control, different charge pressure, too high
Don't think the glow plug codes are relevant (and the car starts fine).
Looks like the vacuum hoses for the turbo actuators are still the prime supect.

I've had a good look at all the technical data and there are a lot of vacuum pipes controlling the various turbo flaps. Most of them look a bit perished. Trouble is that most of them are hard to look at.
Any idea how much the diagnosis, part and repair cost?
