My Ride: 2010 528XI
528i very rough cold start
Proud owner of a 2010 528i xdrive, E60 N52 with 235000 miles. Have developed a very rough idle at first start, like a miss. Have replaced plugs, coils, and injectors. On occasion will receive a code, misfire cylinder ... it will then throw a code misfire all cylinders. Any thoughts or suggestions please

Question:
With that many miles when was the valve cover replaced?When the CCV diaphragm (in the valve cover) is torn or malfunctioning and not holding vacuum the car will idle rough.
When this happens the extreme pressure from the torn diaphragm can force oil out through the valve cover and it will leak in the back near the fire wall.
Check for leaks. While oil leaks appear to be a bad valve cover gasket, the culprit can be a bad CCV on the valve cover.

I share with you my personal work plan for this problem. The list is not in any order of priority, yet the list are actual fixes for people in the past that have had this problem and I captured their issues in a file.
o Obtain the ISTA fault codes first
o If the car idles rough it could be a cracked crankcase breather hose on the valve cover that also runs under the intake manifold
o There is a vacuum leak in one of the other vacuum hoses
o Upper intake boot or lower intake boot is cracked and leaking vacuum
o Hose clamps on upper and lower intake boots are loose
o Oil filler cap seal is worn
o The problem could be bad gas. Fill up with 92 octane and watch
o Cam shaft sensors malfunctioning
o The inside cage on the oil filter could be loose that holds the oil filter in place
o One or more coil packs are bad (even though they are new)
o Cracked plastic valve cover (even though its fairly new)
o Throttle body gasket is leaking
o Bad brake booster or booster valve
o vanos solenoids going bad
o vanos solenoid screens need to be cleaned
o malfunctioning crank sensor
o bad cam sensor
o bad MAF sensor
o manifold gaskets worn
o clogged cat converters
o cracked intake manifold
o bad eccentric shaft sensor
o poor fuel pressure
o Weak fuel pump
o When the battery is low or the alternator is malfunctioning, the car will do crazy things
o Could be a bad oil cap. The rubber seal on the oil cap could be worn
o Could be cracked rubber intake boot upper (near air filter box)
o Could be cracked rubber intake boot lower (near throttle body)
o Hose clamps on both intake boots are loose
o Bad brake booster hose is leaking
o If the car stalls at a traffic light or stutters when accelerating, others have fixed this problem by resetting all the adaptation values (ISTA)
