This pdf of the SIB from 6 years ago should answer your questions. Look at point 4. Your dealer may need to check your software and update it if the car is truly an 03/04 build.

4. For E60/63/64 vehicles produced from 3/04 to 8/04, Bluetooth functionality can be added to most of these
vehicles by updating the vehicle software using CIP 13.2 or higher. After this is performed "Retrofit" of "Tel
Bluetooth" needs to be performed.
What exactly need to be Retrofit in Tel? i have the Motorola v60 in my car but it's not working says "Mobile Phone Not Installed"?

First, find out what your build date is. Is it pre 03/04? If yes, then you need a new TCU if it has not been changed over. If it is built after 03/04, you should be OK.
Find out the status of you BT software in the car. All 2004 E60's shipped to Canada and the US came with BT software turned off. The software needs to be enabled by the dealer or someone with access to the BMW programming interface. Here is where you will spend some $$ with your dealer.
If the car has the software turned off, you will need to have the dealer turn it on. This is the Enable BT part of the SIB. You may qualify for a free software update if you have a warranty issue that is software related requiring an update. Given the age of your car, you may be at the end of any warranty.
Once the software is enabled, then you need to use a compatible phone and you should be good to go.

You should have the updated TCU. Having said that, my E60 was a March 1, 2004 build and it had the old TCU that I ended up having to replace.
You should go to your dealer with the SIB I posted and sit down with someone who has enabled BT in and E60 before and see what will be involved for your car. As I said before, you will likely need the software updated and the BT enabled.
The BT paperwork or card being in the car means nothing. Every E60 had them upon delivery in 2004.
