I have a BT card that came with my car but Bluetooth isn't available in the US due to an incompatibility with BMW Assist.
That's it. Plain and simple. No BT in USA, with or without the card.

I've talked to BMWNA a few times on this issue, including earlier today. They claim that all US MY05 E60 models will be "pre-wired" for BT. However, they're a bit unclear as to whether full functionality will require some sort of extra-cost optional equipment (they recognize that there is not currently a related option code). Also, they've stated emphatically that there will be a MY04 retro-kit available. One person said maybe in October, another said maybe as late as early next year.
At the end of the day, I'm not sure that anyone really knows. My guess is that we'll all just need to wait and see.
