We have grills for other cars, but for the E60, we have a larger grill so it's $49.99 instead of $32.
the $79.99 price point is the SIDE grill for the M5 fender.

Well, normally you already have an M5 
But in the case of those that do not, yes, you have to cut the fender and repaint it. it's not easy/cheap and a good body shop would be the only one that could do it.
