Really?
My guess is she just watched the movie A Few Good Men:
Col. Jessep: Thanks, Danny. I love Washington.
Kaffee: Excuse me. I didn't dismiss you.
Col. Jessep: I beg your pardon?
Kaffee: I'm not finished with my examination. Sit down.
Col. Jessep: Colonel!
Kaffee: What's that?
Col. Jessep: I would appreciate it if he would address me as Colonel or Sir. I believe I've earned it.
Judge Randolph: Defense counsel will address the witness as Colonel or Sir.
Col. Jessep: [to Judge] I don't know what the hell kind of unit you're running here.
Judge: And the witness will address this court as Judge or Your Honor. I'm quite certain I've earned it. Take your seat, Colonel.

In high school, I was a U.S Senate Page for the Democratic party. During that time, I was regularly on the Senate floor among the U.S. Senators at that time.
While you are supposed to call a Senator "Senator", the General here made no insult to Senator Boxer by calling her "Ma'am. Ma'am is a title of respect -- especially to military personnel. I think Sen. Boxer needs to lighten up. There is nothing really wrong with calling a Senator ma'am or sir. I could certainly understand if the genreral called her "Barabara" at the hearing but "ma'am" is fine.
This kind of behavior speaks of an inferiority complex on the part of the Senator. Of course she worked very hard to get the title. But that should not mean that she lor any other Senator should look upon everyone in a lower position in the political hierarchy to be little ants.
U.S. Senators are just like any other people. Some are really nice and some are not. When I was a page, the really nice ones included John Glenn, Bob Graham, Robert Byrd, Bob Dole, Al D'Amato, Tom Daschle, Howell Heflin and believe it or not Jesse Helms (I hated his politics but not him personally). A few of the Senators were downright nasty and arrogant to all they encountered.
Barbara Boxer was a House member then. Ironically, she did not have a particularly bad reputation for treating staff. But being in such a high position where 99% of people you meet kiss you ass can go to one's head. I once worked closely at a law firm with a former U.S. Senator. He had only one term in the Senate and by the time I met him, he was out of the Senate for almost 20 years. He still introduced himself as "Senator" (and then his last name). He actually was a super nice guy. But to him, he defined himself as a Senator even tohugh he had been one for 20 years.

I strongly suspect the General has worked much harder to get to his position in life and and has done much, much more for his country than this lady.
And I say that as a Democrat...
