Please help! Passenger door panel loose

Has anybody had their front passenger inside door panel come "unseated" from the clips along the top of the panel? The top of my panel isn't all the way in. There's about a 1/2" gap there. Near the door lock, the panel is all the way in, but towards the window speaker, there's quite a bit of separation (to the point where I can see the speaker wire inside the door).

How do I fix this? I tried giving it a solid whack with my hand, but that didn't really do anything. Do I have to take off the inside door panel and line it up again? And how do you take off the door panel? Does anyone know the location of the screws, types of screws, general removal procedure, etc?
