Loss of Power on hills and slopes 520d manual

Hi
Can anybody help, ive recently bought a used 520d manual 55 plate, its done approx 60k miles. When driving on the motorway the car loses speed when it comes to a gradient or hill. The power loss is very noticeable. Once the car is back on level ground it has no problem with gaining speed. Can anyone help? is this a common thing or is something not right? Anything i can do to fix it? Would appreciate any comments.

Is the tach climbing, but the speedo not? That is, revving faster, but not speeding up as it should? If so, sounds like a clutch slipping.
