It is slightly more difficult than finding a 550i Auto. But there available, I found them to have less options and yet cost more used. (2008)

Took me about a year to locate a CPO 6MT 530i back in 2007. Options include: Sport, Cold Weather, Leather, Nav. It was off-lease from GETRAG Corp in Sterling Hts, Michigan. I got a good discount since it was in the BMW Center's inventory for 60 days or more - they could not move it. I asked for a CPO and Carfax report, flew out, verified vehicle's condition (per reports), closed the deal and drove home.

I drove one nearly spec'd just like that in October at United BMW in Roswell. It had about 45k on it IIRC. I wasn't looking for a 6speed but that was the only one I had come across in my search.. That said, they are rare.

Not sure if this will help or not, but when I was looking for my current 2008 535i new, I only had two requests...

-6MT
-All Three Packages

Everything else was negotiatable.

I went directly through BMW NA.

There were only 8 available to choose from that were either on a dealer lot, or on the boat over.

Think about it, how many people would spend 70K on a 4 door luxury car and....have to shift their own gears? So not many people will choose that option, surely not the people that the 5 series was marketed towards. Makes sense on a coupe. But not here, traditional manuals are slowly being replaced by SMG type trannys and like the SAT you see in the LCI 5ers. It's truly the best of both worlds. Remember the manual tranny was always the cheaper option than the auto. So you're gonna hafta search far and wide to find a manual and with all your requested options.
