BMW 520d LCI [177] Limp mode

Hi,
As indicated in the title, I have a problem with my car going in limp mode under acceleration (usually on 3rd gear at 3500RPM) every so often. An engine stop / restart and everything is normal until the next limp mode.
I had the codes pulled with a Bavarian Technic tool and they are the following;

4D7F:
Error will not cause a warning light
Error is not currently present
Test conditions have been completed
Short circuit or signal interruption

4CAF:
Error will not cause a warning light
Error is not currently present
Test conditions have been completed
Short circuit or signal interruption

The car had a stage one remap, however the fault codes where there before the remap, meaning that the car had given the same problem to the previous owner at some point in time.
The car was always serviced at BMW and I purchased the car a month after the warranty expired so its curious how the fault was never checked.
Until the remap I never had limp mode, but its possible that the remap made failing components degrade further.
There was also a 3rd fault being shown upon limp mode which indicated a faulty exhaust pressure sensor. This was replaced and the fault never came up again on subsequent limp modes.

Any ideas guys? I really don't wish for an expensive dealer bill at this stage, especially if its a sensor I can change myself. The car has only done 34k miles from new.
