Please look at the attached video clip and pay extra attention at the RPM gauge, notice how the car starts Revving up and down for few seconds then everything goes back to normal. It looks like I'm stepping on the gas paddle but I'm not. This only happens if I don't drive the car for 2 or more days, and it only happens in the morning when I start the car for the first time. I never park my car outside overnight.

The other question, how often do you check your coolant in a cold environment? I typically check once every month and half or maybe two months. Is that too long? Because you will see in the video clip the coolant is low.

I have that sometimes. But is is not that extreme on mine. My guess is contaminated/standing/separated gasoline. Does this happen every time or has this just started now?
Try revving the engine a little bit after start-up (not a lot just a little) and see if the RPM still fluctuates after that. GL

Emissions most likely. The engine is trying to adjust to the O2 sensor which is giving wide swing readings. It might just be the O2 or some other emissions sensor going bad since the issue only happens after the car has been sitting for a while and it's cold.

I asked my SA before, he did mention something about the 02 sensors adjusting since the vehicles been sitting for awhile. My guess is nothing to worry about.

Thanks guys this makes me feel better, I was wondering if a problem is on the way! As I mentioned before it only happens on a cold start after the car has been sitting for at least two days or more. Also not sure if this is related but sometime when I'm backing up the minute I shift to Drive the RPM gauge jumps to around 1200 rpm and I feel the car jumps a bit even when my foot is on the brakes.

That is UNBELIEVABLE!! You actually let your car sit for 2 days or more??? I think most of our cars have this little hiccup when they are warming up!

And if you let your E60 remain idle for too long you will surely need a tricky charger!
