2006 520d engine hesitation

My Ride: e60 520d 2006 model
It's been almost 50000km now that i can't figure out the solution for my issue. I have 225000km done and (except the auto transmission jerky behaviour ) when idling at about 750rpm with P selected on the gear i feel sometimes a small shaking of my car (hesitation of my engine like there is a faulty injector) but just for a second and then back to normal. When i accelarate slowly from a standstill position and at between 1700rpm and 2500rpm there is a big shaking and hesitation like something is clogged and the engine can't breathe. After about 2500rpm it's normal again. I also believe that through that issue my auto transmission became jerky. The only thing i know is that i have 1 burnt glow plug and no other fault code through diagnosis but problem existed before glow plug went out.
MY thoughts are :
- worn torque converter
- swirl flap blocked
- dirty intale manifold
- start of a clogged dpf
- maf or map sensor dead
What else? Please any opinion on that would be appreciated..
