I got mine at the mid night opening of my local Asda, I havent tried online yet but i gather there are issues. They probably dont have enough servers for the demand, I am playing single player at the moment.

Yes it's an optional mission. The first time you log in, it will ask you about a mission with extreme gore or disturbing. You then choose wether you want to play it or not.
This game is freaking intense!
