What is the best way to replace it?
I tried to remove the plastic cover that surrounds the fog without any sucess. I have a E46 M3 and it need just 1 min to do the job, to remove the fog cover by hand and unscrew just one M8 bolt that hold the fog. I've seen some DIY's for normal E60 bumpers, E90 and E82, and to reach the bulb you need to take off the cover underneath the bumper or the wheel cover. This is a 20m job at minimum.

go through the plastic cover.. remove bolts close to the brake duct also... and drop the bottom one also...
it ususally takes me 5 min / side...
but who knows yours is e61 so might be different...

Remove the screws in the front part of the wheel well liner to access the fog lamp assembly. You will find it easier to first steer the front wheel full lock to the side.

WOW, thats quick. I think that it is faster to go through the plastic cover. Like i said above i tried this way but only by hand. Maybe is just the plastics that is strongly attached.
What is the best place to start pulling? Did you pull by hand or do you use any tool?

take screws out and make sure u dont put below tires...
and start from the bottom and up and that will give enough space to lower the lower cover as well.. then it is easy to change the bulb

Take just enough screws off the wheel well liner so that hand can go into area behind fog lamp assembly. I don't remember how many screws, maybe three.

Job done. I removed the fog covers (in the front of the bumper) but the head of the bolts is faced to the inside. This leaded me to the other option by removing the inner wheel cover. I removed four bolts (one at the side, and three at the bottom) and then i pulled the cover but as in the left side there is the windshield water tank i also pulled the cover below the tank to acess the bulb. Is not a clean job and take you at least 10min to change the bulb and reassemble all parts. I hope i don't need to do it again.
