How can you tell a 545 is really a 545 other than the badge?

Just pop the hood and look for a V8... pretty simple in my opinion. Thre only other car it could be with a V8 is a 550i

OP,
Try to race him/ her. If they race, you will quickly know if they have the V8.

My E60 530i Sport has cooling fins on the rear differential.

(Exhaust shown is the Borla catback for the E60 530i.)

Are there styling differences that come standard on a 545i?
The tires are much wider, especially the rears which are also wider than the front ones.
