Default 2008 550i Rear Shock Mount Blow Out

I recently removed & replaced my OEM struts and shocks with Bilstein B4 Touring struts/shocks; I also changed out all the associated hardware like bump stops, dust sleeves, and mounts. I had already replaced all the control arms, tie-rods, sway-bar links, etc. several thousand miles ago. On a test drive immediately after the installation, I hit a bump in the road and the right rear shock blew right through the Lemforder shock mount into the trunk compartment.
I purchased all my parts from FCP Euro and they were great in immediately sending me replacement mount and shock while I had the defective ones removed, replaced, & returned back to FCP Euro.
Yesterday, after driving 1098 miles with the new replacement shock & Lemforder shock mount, IT BLEW OUT AGAIN!! For the second time the right rear shock blew through the mount into the trunk compartment (see attached photo).
I'm sure FCP Euro will be equally helpful again in exchanging the defective parts, but I don't understand why this is happening. I'd love to know if anyone else has had a similar experience or if you have any ideas about a root cause for this repeated breakdown.

are you sure you reused the shock washer from the stock setup. The bilstein instructions might omit the reuse of these.

I believe that is exactly what happened. Took it to a different shop who immediately detected the missing part that should have been saved from the old shock assembly.
